1. Click to Create an Account

To begin the application process, please click the "Create an Account" tab at the top of this page to create an application account. From there, you will be instructed to create a login, password, and verify your email via a message to your email account.

2. Click to Start Application 

You have the flexibility to log in and out of your application account without losing your data as it will autosave. There will be a green check mark for each section once you have completed all required fields.

3. Review and Submit & Pay Application Fee

Once all required fields of the application are complete, you must select "Review & Submit." You will then be instructed to pay the application fee $150 for the first child and $100 for each additional child. 

4. Request/Submit Required Documents

Please be sure that all required documents are provided to the admissions office:

  • Birth Certificate 
  • Baptismal Certificate (for PK4-8 Catholic applicants only)
  • Immunization Record (form 3231)

Please note, you must submit all medical forms to complete the application.

To enroll, you must submit a current physical and Certificate of Vision, Hearing, Dental, and Nutrition Screening to St. Mary’s Academy.

Immunization Requirements 

The State of Georgia and the Archdiocese of Atlanta require up-to-date or completed immunizations for school attendance. All students entering school (through 19 years of age) must be immunized according to the rules and regulations established by the Georgia Department of Human Resources (DHR). The Certificate of Immunization, Form 3231, is the official record of immunization. This certificate is required of each student and must be updated and readily available for auditing or for checking for exemptions in the event of disease outbreak.

Please note that the Archdiocese of Atlanta does not recognize Religious Exemptions.
